The Truth About Filler Migration

"Filler migration" is one of the most talked-about topics in aesthetics right now. TikTok is full of horror stories. Let's separate fact from fear.

What Is Filler Migration, Really?

Filler migration is when hyaluronic acid filler moves from where it was originally placed to a nearby area. It's most commonly seen in the lips, where filler placed in the lip body can migrate above the lip line (creating a "filler moustache" appearance).

It's real. It does happen. But it's much less common than social media suggests, and it's almost always reversible.

What Causes Migration

There are a few genuine causes:

1. Too much product placed too often. The most common cause. If lips are repeatedly filled beyond their structural capacity, there's nowhere for the filler to go except outward. Conservative dosing over time prevents this entirely.

2. Wrong product for the area. Some fillers are thick and designed for cheeks or jawline. Using them in lips can cause migration because the lip tissue can't contain them properly.

3. Poor technique. Filler placed in the wrong anatomical layer, or too superficially, has a higher risk of moving.

4. Natural muscle movement over years. Very slow, gradual migration can occur in any area with repeated muscle activity over many years.

What Migration Looks Like

In lips, migration shows as: - A raised border above the natural lip line - A "shelf" appearance when smiling - Loss of definition in the lip border - Lumps above the upper lip

In cheeks or other areas, it can appear as: - Filler appearing in different positions than originally placed - Irregular shape - Visible lumps

How to Avoid It

1. Choose a qualified practitioner. Dentists, doctors, and nurses with proper training place filler in the correct anatomical layers and use appropriate products. This alone eliminates most risk.

2. Go conservative. Don't chase volume. Start with 0.5ml in lips, see how you look, then decide. A good practitioner will refuse to overfill you even if you ask.

3. Space treatments appropriately. Don't "top up" before you need to. Let the body metabolise some of the filler between sessions.

4. Use the right product. Different hyaluronic acid fillers have different consistencies. Dr Niru uses different products for different areas.

What to Do If You Have Migration

The good news: it's almost always fixable.

Hyaluronidase is an enzyme that dissolves hyaluronic acid filler. It can be injected into the migrated filler to break it down, returning the area to its natural state.

After full dissolution (which takes 24-48 hours), you can choose to have filler placed properly if you want to, or leave it alone.
